Barnsley boss Mark Robins was at a loss to explain the actions of his keeper Luke Steele after his blunder cost the Tykes on Saturday.
The normally exemplary Steele raced off his line against Ipswich and then lost the ball before slipping and watching in horror as the Tractor Boys scored the only goal of the game.
Robins said: "Luke can't explain it, he's made a bad decision. He's come out, left the ball on the touchline, then his mind changes and he's going to struggle to get back."
